Sidelying Shoulder External Rotation Begin lying down on your side; use pillows to support your head so that it is in line with your spine. Optional: Use a weight as indicated by your therapist. Hold a towel under your top elbow. Begin with your hand on your belly. Rotate your arm so your hand goes up toward the ceiling; keep your elbow pinching the towel so it doesn't move from your body. When your hand is pointing up toward the ceiling, pinch your shoulder blades together and hold for one second. Rotate your arm back to lower your hand back to your belly. Repeat.
